Smith & Wesson Inc. announced that they have identified two M&P Shield EZ Pistols on which the hammers manufactured by the supplier were cracked. In those firearms, the hammer failed to fully engage the sear, causing the round to fire, cycling the slide, and potentially resulting in multiple discharges without depressing the trigger.

A settlement has been reached in a class action lawsuit claiming that certain Rossi brand revolvers are defective in that they could unintentionally fire when dropped. The Defendants Forjas Taurus, S.A., and BrazTech International LC (doing business under the brand "Rossi") issued a Warning about these safety concerns in September 2018.

The US Consumer Product Safety Commission reported a voluntary recall of the Alpha Guardian Stack-On Sentinel gun safe as sold by Dick’s Sporting Goods. A bolt malfunction may cause the safe to open without key or combination allowing access to contents.

SIG SAUER, Inc. has determined that a limited number of rifles in the SIG716 DMR, SIG516 Carbon Fiber and SIGM400 Predator models were built with a two-stage SIG SAUER trigger that may have an improperly heat-treated hammer. Over time this could result in a trigger malfunction creating a significant safety hazard.

Daniel Defense is issuing a safety notification related to the Disconnector in the Fire Control Group for Daniel Defense firearms manufactured between April 1, 2016 through March 3, 2017. In a few cases, a double fire condition has occurred with a single pull of the trigger. To determine if a Daniel Defense firearm falls within the scope of this safety notification, consumers should go to the Daniel Defense website.
